This is the only time Goku has gone Kaio-Ken with Super Saiyan. It's not stated anywhere but the general consensus is that thanks to being dead and not having to worry about dying from strain he was able to enter Kaio-Ken while in Super Saiyan and not worry about the effect on his body. It's never used prior or after. And most will be quick to state that it isn't cannon just filler. Still awesome! :D

This happened in between the cell saga and buu saga while Goku was dead. To give you an idea on how strong Pikkon is, he managed to Knock Out Super Perfect Cell in 2 shots

Great fight for a filler right? Technically they both lost cause they touched the ceiling. But Pikkon was probably stronger
