It stands for "straight trans for trans", so I guess both of the characters being trans, but one of them being a different gender identity from how they are in canon (I had to clarify with the requester because I wasn't 100% sure)

A note on the option for Yuu and Touko from Bloom Into You - it looks like they also have a manga epilogue where they appear as adults, so I'd be willing to do an NSFW illustration; requester mentioned having one or both of them be trans!
