I'm sorry but we can't except that document as proof of consent π... while it is signed and dated by both parties involved I'm afraid you failed to have a witness present for the signing so it could be properly notarized, not to mention You've both failed to initial beside each prior agreed upon sensual act & position, and finally you've failed to fill out the last page in triplicate... lol but jokes aside I do agree that consent is extremely important I just think that it might be hard to convey that in "certain" situations...
